They want to know the name of your supervisor there and that person’s job title. (Like, “John Smith, Accounting Manager”)
Usually its the person that is your direct supervisor. Its usually the person who trains you and the person who is in charge of your department. If you dont work in a big company, its usually just the boss of everyone.
