I presume this is for a former job? They want the name and TITLE (not tittle) of your immediate supervisor. If you don’t know their title, leave it blank.

I’m assuming you’re in the part where you talk about your current or previous jobs.
They want to know the name of your supervisor there and that person’s job title. (Like, “John Smith, Accounting Manager”)
Usually its the person that is your direct supervisor. Its usually the person who trains you and the person who is in charge of your department. If you dont work in a big company, its usually just the boss of everyone.
